One of the World's Leading Expert on Root Cause Failure Investigation
Paresh Haribhakti has spent more than three decades answering the question that keeps plant managers awake at night: why did this fail, and what do we do now. As Managing Director and Chief Metallurgist of TCR Advanced Engineering, he has personally led or supervised more than 10,000 root-cause failure investigations for refineries, fertilizer complexes, power stations, and process plants across India and the Middle East, establishing him as one of the world's leading authorities in failure investigation and root cause analysis.
He holds a Master of Engineering in Material Technology and a Bachelor of Engineering in Metallurgical Engineering, both from Maharaja Sayajirao University of Baroda. His work spans failure investigation and root cause analysis, fitness for service (FFS) assessment per API 579-1/ASME FFS-1, remaining life assessment (RLA) of boilers, heaters, reformer tubes, and pressure vessels, and corrosion engineering across oil and gas, refining, petrochemicals, fertilizers, and power generation.
He is the architect of AiOM (Asset Integrity Optimization and Management), TCR Advanced's proprietary platform for predictive, data-driven asset integrity, co-author of the ASM International monograph "Failure Investigation of Boiler Tubes: A Comprehensive Approach" (2019), and a contributor to ASM Handbook, Volume 11A on failures of boilers and related equipment (2021).
Engagements That Define the Standard
A selection of assignments illustrating the range and consequence of Paresh Haribhakti's failure investigation and fitness for service work.
Reliance Industries Limited — Jamnagar
Led TCR Advanced's metallurgical investigation into the fire incident at the VGO-HT2 plant of Asia's largest grass-root refinery, assessing integrity of pipelines, flanges, heat exchangers, and reactors following the event.
Major Refinery — Isomerization Reactor
Led the fitness-for-service assessment of a hydrogen-service isomerization reactor to API 579/ASME FFS-1, avoiding premature replacement and delivering a direct cost saving of over USD 15 million to the plant.
ITER Project — International Consortium
TCR Advanced's metallurgists and fracture mechanics specialists conducted a detailed study of the nuclear reactor outer shield for the ITER project, a collaboration of eight countries headquartered in France.
Saudi Aramco & SABIC, Kingdom of Saudi Arabia
Conducted RO plant corrosion investigation, furnace reactor flange failure analysis, and Omega-method RLA of furnace tubes per API 579 for Saudi Aramco; delivered failure investigation, RLA, and FFS for high-pressure, high-temperature components at SABIC.
QAFCO, Qatar
Performed RLA, failure investigation, and condition assessment of high-pressure, high-temperature components, including on-site repair decisions, for the world's largest integrated fertilizer plant.
Godrej Industries — Valia, Gujarat
Directed the complete health assessment of an entire hydrogen plant relocated as a used unit from England to Godrej's Valia facility, one of TCR Advanced's most comprehensive single-site integrity assignments.
Innovation in Asset Integrity: AiOM and Beyond
Paresh Haribhakti leads TCR Advanced's in-house team of fracture mechanics specialists, corrosion scientists, and software engineers in building tools that shift industry from reactive maintenance to predictive, data-driven asset management.
AiOM — Asset Integrity Optimization and Management
A software platform integrating damage-mechanism knowledge, maintenance history, and operating parameters to present availability, reliability, total downtime, maintenance cost, and failure frequency in one decision-ready view, helping process plants extend asset life at optimum cost, reliably and safely.
ARTiS — Automated Reformer Tube Inspection System
An indigenously developed robotic crawler for ultrasonic inspection of reformer tubes, capturing wall-thickness attenuation, diameter, and bowing angle at every location and presenting results in digital format for rapid NDT evaluation.
Microstructure Characterizer (MiC) Software
An AI-assisted image-processing tool used by research laboratories worldwide, including National Oilwell Varco (USA), for grain size, coating thickness, and phase composition characterisation.
